If you would like, we have two separate forms when if come to children. The
first which covers youth(13-17) is a simple parental consent form. The
second is a chaperon agreement form which states that the children(12 and
under) will be in the constant supervision of the signed adult and holds our
event harmless, and holds the adult responsible in case of any mishaps or
damages incurred. We'd be more than willing to share this with you.
