[Penguicon-Concom] Attendee Release Form

I think there also needs to be something about children needing to be 
accompanied by an adult at all times and membership being revoked if 
children are left to run wild.  We had a bunch in consuite last year 
that were totally out of control and there didn't seem to be any 
responsible adult around to make them behave.  They kept trying to 
interfere with the LN2 ice cream and could have gotten injured.
