[Board] P-con reg page

Hi Ron, I'm looking at the reg form in IE right now (cuz that's how I
get to through the web proxy at my customer), and yeah, it's messed
up.

I've attached a word document (yeah, I know, boo hiss, but I'm looking
at this at work, and this is what a lot of folks have to use,
regardless.)

So, take a look and see.  You should be able to pull this up in OO if necessary.
